Fatherhood Community Health Worker
The Fatherhood Community Health Worker (CHW) provides care coordination, referrals and follow-up,
and birth and parenting education for Fathers to achieve appropriate child health, social, and emotional
development, and milestone achievement. Reports to the Fatherhood Program Manager.
Doula
The Doula provides prenatal, delivery, and postpartum care to pregnant people who screen as high risk for adverse health and birth outcomes. The Doula will provide culturally responsive care, education, and support through home visits during and after birth. The Doula will promote healthy pregnancies, breastfeeding, and parent-infant attachment. Visits will focus on improving
health and birth outcomes and building resilience and support for families.
